What are new students required to bring in order to register?

Birth Certificate
Immunization Records (all students entering 6th grade and above must have a TDAP shot)
Virginia Physical
Custody Agreements (if applicable)

Fees at SMMS.

Planner $5.00

In addition, parents/guardians will need to complete the following forms, which can be completed prior to arrival at SMMS.

Acceptable Computer System Usage Agreement
Virginia Lions Club Screen Form (7th graders only)
Student Over the Counter Medication Permission Form
Student Health Information Form
